Advance care planning (ACP) is for everyone, regardless of age or state of health. ACP is a series of voluntary conversations that allow you to discuss, reflect and decide on the type of healthcare treatments you may or may not want. By voicing and documenting your preferences now, you will guide the healthcare team and your loved ones to make healthcare decisions on your behalf, in your best interests, if you are unable to do so yourself in the future.
These topics are usually discussed during ACP:
- Your personal values
- Worries/ concerns about your health
- Things that make your life meaningful
- Your chosen spokesperson and his/ her role in your care
